• Patient names or initials tied to a real case.
• Dates of birth, ages combined with unique facts, medical record numbers, account numbers, or visit numbers.
• Addresses, phone numbers, email addresses, photos, or employer/facility identifiers tied to a patient.
• Specific dates, locations, or rare details that could identify a real patient.
• Any information from a real chart that your school, employer, clinical site, or facility policy does not allow you to share.

• Use fictional scenarios created for study.
• Use broad details, such as "adult patient with heart failure" or "post-op patient with new shortness of breath."
• Change or remove dates, locations, and unusual facts.
• Ask concept questions, such as "why does potassium matter with furosemide?"

If you are not sure whether a detail could identify a real patient, do not enter it. Ask your instructor, clinical site, employer, or privacy officer how to handle the situation.
